Participant Interactive Journal
Moving Forward
This Interactive Journal is an effective resource for
guiding individualized service planning through the
treatment and continuing care process. Treatment
team and participant work together to identify and
set goals targeted to specific dimensional needs and
later reevaluate progress, allowing the participant
to expand on and adjust goals.

blue boxes. Secondary dimensional subjects for each Journal are
indicated by the yellow boxes.
For example, the "Getting Started"
Journal primarily supports Dimension 4, Readiness to Change, while
it secondarily covers Dimensions 3, 5 and 6.
